The science
We would rather tell you where the evidence is strong, where it is thin, and where it simply does not exist yet.
Serum 25-hydroxyvitamin D is the marker. Below 12 ng/mL is deficient; 12-20 ng/mL inadequate; 20 ng/mL or above adequate for most people. Above 50 ng/mL raises concern for adverse effects.
NHANES 2011-2014 data put roughly 5% of Americans at risk of deficiency and about 18% at risk of inadequacy — with a marked disparity: 17.5% of non-Hispanic Black Americans versus 2.1% of non-Hispanic White Americans at risk of deficiency.
The adult tolerable upper intake level is 4,000 IU/day. Bolus regimens are dosed at intervals and are appropriate only under supervision with laboratory monitoring — which is how we run them.
Toxicity comes from supplementation rather than sun exposure: excess causes hypercalcaemia and hypercalciuria, typically at 25(OH)D above 150 ng/mL, and severe cases involve renal failure and arrhythmias. Some large-bolus trials have also reported increased fall and fracture risk.
Who commonly considers this: People with lab-confirmed low 25(OH)D, limited sun exposure or darker skin, fat-malabsorption conditions, obesity, or bariatric surgery history.
