The science
We would rather tell you where the evidence is strong, where it is thin, and where it simply does not exist yet.
NAD+ is a coenzyme central to glycolysis, the citric acid cycle and oxidative phosphorylation. It is also a required substrate for sirtuins and for PARP DNA-repair enzymes — meaning NAD+ is consumed by these processes, not merely recycled.
Work published in Cell Metabolism showed that the NADase enzyme CD38 rises with age and drives age-related NAD+ decline and mitochondrial dysfunction through a SIRT3-dependent mechanism. NAD+ falls largely because it is being destroyed, not simply under-produced.
Oral precursors such as nicotinamide riboside and NMN reliably raise blood NAD+ metabolites and have good safety records in randomised trials. Consistent functional or anti-aging benefits in healthy people have not been demonstrated, and IV NAD+ specifically has minimal pharmacokinetic and outcome data. This is an area of active research, not settled science.
In a published tolerability study, all participants receiving 500mg IV NAD+ reported symptoms including abdominal cramping, nausea, increased heart rate, throat pain and chest pressure — and those symptoms stopped immediately when the infusion was slowed or paused. Participants who controlled their own rate averaged about 97 minutes. Speak up the moment you feel anything; slowing the drip is the answer.
