The science
We would rather tell you where the evidence is strong, where it is thin, and where it simply does not exist yet.
ACOG recommends pyridoxine as first-line therapy for nausea and vomiting of pregnancy, alone or with doxylamine. NIH ODS notes trials in which 30-75 mg/day oral pyridoxine significantly reduced nausea, with roughly a 70% reduction when combined with doxylamine. Note that this evidence base is oral, not intravenous.
Its active form, pyridoxal 5'-phosphate, is a cofactor in over 100 enzyme reactions including neurotransmitter synthesis — the proposed route by which it modulates nausea signalling.
These support red blood cell formation, one-carbon metabolism, collagen synthesis and non-heme iron absorption — demands that rise modestly in pregnancy. Folate specifically should come from a prenatal vitamin under obstetric guidance.
B6 has a tolerable upper intake level of 100 mg/day, and chronic high-dose pyridoxine causes progressive sensory neuropathy. Doses must stay within ACOG's range and account for the prenatal vitamin you are already taking. We calculate this with you.
Who commonly considers this: Pregnant individuals experiencing nausea or difficulty maintaining fluid intake who have discussed IV support with their obstetric provider.
