What the research actually shows

We would rather tell you where the evidence is strong, where it is thin, and where it simply does not exist yet.

Why injection exists as an option

Dietary B12 requires gastric acid, then intrinsic factor from stomach parietal cells, for receptor-mediated absorption in the distal ileum. That pathway saturates: oral absorption is around 50% at 1-2 mcg doses but only about 1.3% at 1,000 mcg. Injection bypasses it entirely.

But high-dose oral often works too

A minor passive-diffusion route of roughly 1% explains why high-dose oral B12 can work for many people, and trials generally find high-dose oral comparable to injection for uncomplicated deficiency. Injection's real advantage is in malabsorption — which is a reason to find out which situation you are in.

Deficiency does not always look like anaemia

Neurological signs such as numbness, tingling, or changes in gait or memory can occur without anaemia. Body stores of 1-5mg mean symptoms may take years to appear.

The energy question, answered honestly

NIH ODS states that B12 'appears to have no beneficial effect on performance in the absence of a nutritional deficit.' If your B12 is normal, do not expect a lift. If it is low, correcting it can matter a great deal. Also worth knowing: there is no evidence that absorption differs between methylcobalamin and cyanocobalamin.

Who commonly considers this: People with confirmed or suspected low B12 — vegans and vegetarians, adults on long-term metformin or acid-reducing medication, people with pernicious anaemia, atrophic gastritis or bariatric surgery history, and older adults.

Safety, screening & contraindications

  • Injectable B12 is a prescription medication requiring a licensed prescriber. There is no tolerable upper intake level and it is generally considered safe even at high doses.
  • Status is best confirmed by blood testing rather than symptoms alone.
  • Supplementing B12 can mask folate-deficiency anaemia while neurological damage progresses — another reason to test.
  • Tell us about metformin, PPIs or H2 blockers, all of which lower B12 status over time.
