The dual mechanism

By engaging both incretin pathways, tirzepatide enhances glucose-dependent insulin secretion, slows gastric emptying and reduces appetite.

What SURMOUNT-1 found

In adults with obesity or overweight without diabetes over 72 weeks, mean weight reduction was approximately 15.0%, 19.5% and 20.9% at 5mg, 10mg and 15mg weekly respectively, versus 3.1% for placebo on the treatment-regimen estimand. The efficacy estimand reported up to around 22.5% at the highest dose.

Side effects mirror the class

Nausea, vomiting, diarrhoea, constipation and reduced appetite, generally dose-dependent and most pronounced during titration.

Serious risks

Pancreatitis, gallbladder disease, and hypoglycaemia when combined with insulin or sulfonylureas. Beyond obesity, tirzepatide is also approved for type 2 diabetes and for obstructive sleep apnoea in obesity.

Who commonly considers this: Adults with obesity, or overweight with a weight-related comorbidity, following medical evaluation.

Safety, screening & contraindications

  • Prescription-only. Requires supervised titration and ongoing monitoring.
  • BOXED WARNING for thyroid C-cell tumours. Contraindicated with a personal or family history of medullary thyroid carcinoma or MEN 2.
  • Compounded tirzepatide is not FDA-approved. Ask us what you are being prescribed.
  • Tell the prescriber about insulin or sulfonylurea use because of hypoglycaemia risk.
