How it works

Semaglutide is an incretin mimetic: it augments glucose-dependent insulin secretion, suppresses inappropriate glucagon, slows gastric emptying, and acts on hypothalamic appetite circuits to reduce hunger and food intake.

What the STEP 1 trial found

In 1,961 adults with overweight or obesity without diabetes, over 68 weeks on 2.4mg weekly plus lifestyle intervention, mean weight change was -14.9% versus -2.4% for placebo. Roughly 69-79% achieved at least 10% loss and 51-64% achieved at least 15%, against 12-27% and 5-13% on placebo.

The side effects are mostly gastrointestinal

Nausea 44.2%, diarrhoea 31.5%, vomiting 24.8%, constipation 23.4%. GI events led to discontinuation in 4.5% versus 0.8% on placebo. Titration exists specifically to manage this.

Set expectations about stopping

Weight regain is typical after discontinuation, so this is best understood as ongoing therapy rather than a course you complete. Serious risks include pancreatitis, gallbladder disease and reported vision changes.

Who commonly considers this: Adults with obesity, or with overweight plus a weight-related condition, who qualify after medical evaluation.

Safety, screening & contraindications

  • Prescription-only. Requires prescriber evaluation, dose titration and ongoing monitoring — not a menu selection.
  • BOXED WARNING for thyroid C-cell tumours seen in rodents. Contraindicated in anyone with a personal or family history of medullary thyroid carcinoma or MEN 2.
  • Compounded semaglutide is not FDA-approved, and the FDA has warned about compounded GLP-1 products and dosing errors. Ask us directly what you are being prescribed.
  • Discuss all medications with the prescriber, particularly insulin or sulfonylureas.
